The Joint Committee on Vaccination and Immunisation (JCVI) has decreed that all Health & Care workers should be among the first group of people to receive a Covid vaccination.

The Avisford Medical Group welcomes new patients.

We will accept new patients within our catchment area.

Catchment Area includes : Aldingbourne, Barnham, Bilsham, Binsted, (some of) Bognor Regis, Boxgrove, Climping, Eastergate, Elmer, Felpham, Fontwell, Ford, Middleton, Oving, Slindon, Woodgate and Yapton.

If you are unsure whether you are in our catchment area please ask our reception staff.

From January 2015, the NHS made provision for GP practices to register patients who live outside their practice area (without any obligation to provide home visits) - but entirely at their own discretion. Avisford Medical Group has a policy NOT to allow any out-of-area patients to register or to remain as existing patients if their home address is outside our catchment area.

The registration of a new patient involves the completion of a number of forms.

A supply of these can be found in the Reception areas at both Yapton and Middleton.

Legislation dictates that new patients must provide evidence of their identity – but that no-one should be refused treatment while such identification evidence is awaited. Please refer to relevant notes below.

If you have moved here from another area then you will need to register with us. We will then arrange for the transfer of all your old records from your previous GP.

If you wish to change from your current GP and transfer your care to Avisford Medical Group, you will need to provide the following:

  • Proof of your current address (e.g. utility bill, bank statement)

  • Photographic proof of identity (e.g. passport, photo driving license)

  • Name and address of your last GP practice

  • NHS Number (if you are unsure of this ask your last GP practice)

  • Post code of your previous address

You will be encouraged to have a new patient check so that we can start to assess your requirements and best plan your care. Please book this as soon as you can or before you run out of medication.
